Terms of use

Effective 2026-10-04

Owner-approved 2026-10-04; no counsel review. This is an informational compilation, not legal advice or a legal determination.

Agreement

These terms govern your use of oculirx.com, the OculiRX screening and the provider portal, which are operated by Centaris Health Inc., a Delaware corporation ("we"). By using them you agree to these terms. If you do not agree, do not use them.

What the service is

OculiRX is a screening-grade distance visual-acuity measurement that a licensed eye-care professional reviews. OculiRX:

  • does not diagnose any condition
  • does not replace a comprehensive eye exam
  • does not render a clinical decision
  • does not measure refractive error, color vision, depth perception, contrast sensitivity, or visual field
  • does not generate eyeglass or contact-lens prescriptions

Nothing on this site is medical advice. Any clinical decision is your eye-care provider's. If you have sudden vision loss, eye pain or an eye injury, seek in-person care right away, and call 911 in an emergency.

Who may use it

Prescription use only (Rx). Federal law restricts this device to use by or on the order of a licensed eye-care provider. Access is gated by a provider-issued credential.

Patients may take the screening only through the link their provider sends them, and only for themselves. Provider and practice accounts are created by a practice administrator; there is no public sign-up.

Provider responsibilities

  • Hold the licenses your practice requires where your patients are located.
  • Review each result yourself and make any clinical decision.
  • Follow the laws and board rules that apply to remote eye care in your state.
  • Keep your sign-in details and two-factor device secure.

Acceptable use

You agree not to:

  • take a screening for someone else, or give false information;
  • access accounts, records or links that are not yours;
  • interfere with, probe or overload the service, except under our security policy;
  • copy, reverse engineer or resell the service.

We may suspend access that breaks these terms or puts the service at risk.

Ownership

The service, its software and content belong to us or our licensors. These terms do not give you any ownership rights.

Privacy

Our privacy notice explains what we collect and how we use it.

Disclaimers and limits

The service is provided "as is" and "as available". To the extent the law allows, we disclaim implied warranties, and we are not liable for indirect or consequential losses arising from use of the service. Nothing in these terms limits liability that cannot be limited by law.

Changes

We may update these terms. We will post changes on this page and update the effective date above. Questions: privacy@oculirx.com.

OculiRX is a provider-supervised, screening-grade visual-acuity test. It measures distance visual acuity for review by a licensed eye-care professional and does not diagnose disease, perform refraction, or replace a comprehensive eye exam. Rx only. A formal validation study is proposed and has not yet been conducted; we do not publish accuracy figures until the data supports them. For emergencies, call 911.
